A single loose wire caused the blackout that led to the Dali container ship crashing into the Francis Scott Key Bridge in Baltimore last year, an investigation has revealed.

Six construction workers were killed when the accident happened, and investigators said the quick actions of the ship’s pilots and the Maryland Transport Authority undoubtedly saved others.
