MAIB issues flexible hose fire warning

The MV Finnmaster was departing Hull in the UK on 19 September 2021 when fire broke out in the auxiliary engine room. The fuel source of the blaze was identified as coming from a flexible hose that had overheated following an exhaust gas leak from one of the engine’s turbochargers.

The investigation found that the hose installation contravened guidance, as it was too long and had been routed close to hot surfaces. Its installation has not been surveyed or approved.

Furthermore, despite being replaced a number of times in the 15 years since its initial installation, the material, length and routing of the hose had remained the same throughout and the risk it posed had not been identified or mitigated.

Recommendations

The MAIB is warning that the risks associated with modifying safety equipment should be considered before and during installation work.

It also advises that metal pipes might be a safer option, particularly if the positioning of existing equipment is such that a long hose is required. Isolation values should also be fitted if flexible hoses are to be used.

The accident remains under investigation.
